This is where you can find all the bio forms, and explanations of them! Don't forget to update.
Clan Pokemon Bio Sheet
Name: Their current name.
Past and Future Names: Their past and future names. You do not need to put leader name here.
Species: Obvious.
Gender: Male or female?
Level: Kits are level 1, apprentices level 5, warriors are at least 20, healers are at least 20, elders are 75-90, usually. Elders can be younger than that, however.
Clan: Their Clan.
Type: Their type(s).
Ability: Their ability.
Rank: Kit, Apprentice, Healer Apprentice, Warrior, Healer, Deputy, Leader, or Elder?
Moves: Look on http://serebii.net/pokedex-dp/ to see what moves your Pokemon knows at whatever level it is. Put them here, and update this as time goes on. If your Pokemon is not its normal type, please ask me (Bunny) which level-line to use. For example: If you have a Charmander who has been turned into a water type, I may tell you to use the Squirtle move line. Of course, if you have a flying-type Bulbasaur without wings, your Pokemon can't learn moves like Wing Attack because it doesn't have the necessary body parts.
Sprite: You must have a sprite. If you don't and can't sprite one, have somebody sprite it for you. You can draw a picture, but somebody must make you a sprite based on said picture.
Personality: At least eight good sentences about your Pokemon's personality. Is it nice or mean? What are its favorite things? Does it do anything differently from other Pokemon? If so, what? Stuff like that.
Description: Even if you have a sprite already, a description of your Pokemon always helps. This must be six sentences, at least. More would be appreciated.
History: Your Pokemon's history. Depending on the rank, it should be more or less. Kits are about four, while Warriors are about eight. Please update this. You can update in bullet points, although you could in sentences as well.

Special Power: This is your stray's special power. A special power can be an odd thing they can do. Invisibility, time travel, etc. It can't be too god-modish like mind-control, but something like mind-reading would be fine. Each power can only be used once, unless a stray has a kit. The kit can have a similar or the same power. They don't necessarily have to though.

LifeClan/DeathClan Pokemon Bio Sheet
Name: Their current name.
Past Names: Their past names.
Species: Obvious.
Gender: Male or female?
Level: The level they were when they died.
Allegiance: LifeClan or DeathClan?
Past Clan: Their past Clan. If none, say so.
Type: Their type(s). Same as when they died, unless something special happened at their death to change it. For example; a Pokemon who died in a fire might have adapted the fire type in addition to or instead of their old one.
Ability: Their ability when they died. If their type changed, their ability might have as well.
Old Rank: What rank were they when they died?
Moves: List all of the moves they had, plus two special moves of your choice from below. There are five moves to choose from for both Death and LifeClan.
LIFECLAN: Future Sight (Healers only, to predict the future and make prophecies), Roleplay (Able to change shape to whatever you want and then appear in front of one to four living, awake Pokemon), Teleport (...enough said), Reflect (allows you to show anybody the past, and see it yourself), Gravity (allows you to stop time for a short amount of time, and if you have physical contact with another Pokemon, living or dead, they are not frozen)
DEATHCLAN: Nasty Plot (Healers only, to help create evil schemes and fake prophecies), Dream Eater (Gives nightmares to any living, sleeping Pokemon, and can be used to talk a Pokemon into doing bad things), Teleport (...whaaaat?), Haze (Blocks a Pokemon's view of good and bad), Perish Song (Lures a dead Pokemon to the underworld to be held prisoner and tortured, can only be used on a Pokemon that has just died)
Sprites: Their old sprite, plus a LifeClan/DeathClan one. LifeClan Pokemon are tinted blue and starry, while DeathClan are red and black. These colors only appear to living Pokemon.

Tame bio sheet
When making a tame, all elements are the same, with the exeption of the Trainer and Teammates slots. Also, try to remember that Tames are born from eggs if not born in the wild.
Age: You don't have to be exact. Just say something simple like 'Newborn' 'Young' 'Adult' or 'Very old' x3 We don't know how long Pokemon live.
Trainer: The human who has captured or is in ownership of the Pokémon.
Teammates: The other Pokémon that travel with the trainer, if any.
